Assessment Tools do not take into account other considerations including, but not limited

consideration of each patient.
Renal function should be assessed by calculating the creatinine clearance (CrCL) prior to
initiation of treatment with Pradaxa. Estimation of renal function using the Cockcroft-Gault
formula is recommended, rather than eGFR.1
Treatment with Pradaxa in patients with severe renal impairment (CrCL < 30 ml/min) is
contraindicated.1
While on treatment renal function should be assessed in certain clinical situations when it is
suspected that the renal function could decline or deteriorate (such as hypovolaemia,
dehydration, and with certain co-medications).1
Renal function should be assessed at least once a year in patients aged>75 years and in any
patients with moderate renal impairment.1
